Knows the dietary therapies available to reduce disease and
symptoms (e g for intermittent obstruction l giving a liquid or low fibre
diet)

Is able to investigate the causes of a small bowel arterial or venous
infarction

Can identify other co-existing vascular problems
Can choose the appropriate route for nutritional support (EN or PN)

spontaneously

Is able to prescribe appropriate pain relief (often with the pain team)
Can assess fluid losses and thus give appropriate fluid replacement

Is able to arrange the appropriate tests for mapping the remaining gut
(both that is in and out of circuit)

Gives appropriate psychological care with the psychological medicine
team

Helps a patient to be physically and emotionally well so they are able
to tolerate more surgery if necessary or be able to cope at home

Can feed into defunctioned gut (fistuloclysis) when appropriate

Nutrition and Diabetes

Medical Nutrition Therapy for type 2 diabetes 1. Caloric intake (balanced with caloric expenditure) improves glycaemic control 2. Weight loss- Aim for 5-10% weight loss with Exercise (Improved glycemic control/ Weight control/ Reduction in co-morbidities (HTN, CV disease, and depression) /Improved QOL Meal planning (calorie counting/ meal replacements Consider medications (sibutramine/ Rimonabant, a selective cannabinoid type 1 (CB1) receptor blocker) Surgery if diabetes and BMI is ≥35 kg/m2. Diabetic meds: Insulin and thiazolidinediones cause weight gain, metformin and exena-tide (incretin mimetic) therapy cause weight loss. 3. Consistency in day-to-day carbohydrate intake at meals and snacks Carbohydrate counting — Usually 50% of total calories. Aim for 30-45g meals and 15-20 g for snacks. Overall < 130 g/d 4. Nutritional content A diet that includes carbohydrates from fruits, vegetables, whole grains, legumes, and low-fat milk is encouraged Saturated fat should be less than 7 % of calories and there should be minimal trans fat. Total cholesterol < 200 mg daily. Renal impairment: No CKD: Dietary protein of 15 to 20 % of calories. Early CKD: Reduction of protein intake to 0.8 to 1.0 g/kg body weight per day Late CKD: Reduction to 0.8 g/kg body weight per day High protein diets (>20 % of calories) are not recommended f Fibre intake should be > 14g/1000 calories daily Sodium intake < 2300 mg per day, or <2000 if heart failure5. Timing of meals and snacks- Insulin regimes require a consistent food intake.
